Patient forms

The clipboard questionnaire, filled before the patient sits down

Every new patient answers the same questions: allergies, medications, conditions, then signs a consent. On paper those answers stay in a folder, and the doctor asks again. In Relatica the clinic builds its own forms or starts from a template for its type of practice. The patient fills the form at home from a link, on the clinic's tablet, or the receptionist ticks the answers while they talk. Answers about allergies, medications and conditions are added to the medical record.

How a form gets filled

One form, three ways to fill it.

  1. 1

Start from a template or a blank form

There is a health questionnaire for each clinic type and two consents, for treatment and for photos. A copied template becomes your own form to change. The templates follow common paper forms; have your doctors read them before you use them.

  2. 2

    Ask what you need

Questions can be yes or no, one or several from a list, short or long text, a date, a signature drawn with a finger, or a plain text block. Any question can be required, can ask for details after "yes", or offer "other" with a text field.

  3. 3

    Decide where each answer goes

An answer can go to allergies, conditions, medications, medical notes, the patient alert, the date of birth or the email. Answers are added to what is already there and never overwrite it.

  4. 4

Send it, or hand over the tablet

Send a link by SMS or email, valid for 14 days. Or open the form on the clinic tablet with a QR code, valid for three hours and one use. Or fill it at the desk, and the record says the clinic entered it.

  5. 5

    Read and print the answers

Answers are kept with a copy of the form as it was sent, so changing the form later does not change old answers. Print one or save it as PDF, with the signature.

What the clinic controls

Forms live in Settings, the filled ones on the patient's Documents tab.

Your own forms
As many as you need: a first-visit questionnaire, a consent per procedure, a photo consent.
Three languages
Write each form in English, Greek and Russian. The patient sees their language; a missing translation falls back to one you wrote.
Who reads the answers
Health answers are open to clinical roles. The front desk can open consents and paperwork but not the medical answers.
Alerts
An answer mapped to the alert appears in red on the patient header, where reception and the doctor see it.
Archive
Retire a form you no longer use; answers already given stay on the patients.

What forms do not do yet

Checked against what clinics ask for.

  • No conditional questions or sections that appear depending on an earlier answer.
  • No number or file-upload questions.
  • Forms are not sent automatically with each booking, and there is no reminder to fill one in.
  • A signed form is not filed into Documents as a PDF by itself; print or save it from the answers.

On every plan, including Solo

Forms are part of every plan, including Solo. A link sent by SMS uses the plan's SMS allowance; email, a copied link and the tablet cost nothing.

Questions about patient forms

Do we have to build forms from scratch?

No. Copy the template for your clinic type and change what you want. A clinic that has made no forms still gets the built-in questionnaire for its type.

Can the patient sign the consent?

Yes. A signature is a question type: the patient draws it with a finger on a phone or tablet, and it is shown on the printed form.

What if a patient has no smartphone?

Hand them the clinic tablet, or fill the form at the desk while they answer. The record shows that the clinic entered it.

Will the answers overwrite what the doctor wrote?

No. Mapped answers are added to allergies, conditions and medications; nothing already on the record is replaced.

Can Greek and Russian patients fill the same form?

Yes. Each form has texts in three languages, and the patient sees the one they read.

Who can see the answers?

Practitioners and managers see the health answers. Reception sees consents and other paperwork, not the medical answers.
